formou skenu objedn\u00e1vky alebo zmluvy (s prekryt\u00fdmi osobn\u00fdmi a finan\u010dn\u00fdmi \u00fadajmi) alebo preberacieho protokolu alebo (ak to nie je mo\u017en\u00e9) \u010destn\u00fdm prehl\u00e1sen\u00edm objedn\u00e1vate\u013ea;<\/li>\n<li><strong>inform\u00e1cie o diele:<\/strong> n\u00e1zov, typ diela (plastika, mozaika, objekt, font\u00e1na, minca, medaila, po\u0161tov\u00e1 evidencie ocenenia alebo ohlasu vyu\u017eite pr\u00edslu\u0161n\u00fd formul\u00e1r):\n<ul>\n<li>dokument\u00e1cia pre ocenenie: n\u00e1zov ocenenia a kateg\u00f3rie, v ktorej bolo ocenenie
