prehl\u00e1sen\u00edm objedn\u00e1vate\u013ea alebo z\u00e1v\u00e4zn\u00fdm p\u00edsomn\u00fdm vyjadren\u00edm ofici\u00e1lneho z\u00e1stupcu zverej\u0148uj\u00facej in\u0161tit\u00facie a pod., ak vzniku, technika, rozmery (rozsah), ob\u00e1lka, tir\u00e1\u017e, rub aj l\u00edc tituln\u00e9ho listu a obsah v pr\u00edpade dizajnu ob\u00e1lky alebo fulltext v pr\u00edpade dizajnu publik\u00e1cie, miesto a rok prv\u00e9ho zverejnenia, vydavate\u013esk\u00e9 \u00fadaje, n\u00e1zov podujatia prv\u00e9ho zverejnenia (ak sa jedn\u00e1 o dizajn katal\u00f3gu k podujatiu \u2013 v\u00fdstave, a pod.), n\u00e1zov organiz\u00e1tora podujatia;<\/li>\n<li>s\u00fa\u010das\u0165ou dokument\u00e1cie je aj <strong>dokument\u00e1cia k oceneniam <\/strong>a<strong> ohlasom<\/strong>, pokia\u013e maj\u00fa by\u0165 s\u00fa\u010dasne evidovan\u00e9 s v\u00fdstupom umeleckej \u010dinnosti, v pr\u00edpade dodato\u010dnej evidencie ocenenia alebo ohlasu vyu\u017eite pr\u00edslu\u0161n\u00fd formul\u00e1r:\n<ul>\n<li>dokument\u00e1cia pre ocenenie: n\u00e1zov ocenenia a kateg\u00f3rie, v ktorej bolo ocenenie udelen\u00e9, miesto a d\u00e1tum udelenia ocenenia,
